Submit Records Requests in Person

To obtain printed copies of your medical records in person, please visit the Medical Records office located inside Griffin Hospital.

Submit Records Requests Through our Online Request Tool

We now have a fully online records request tool that ensures security through identity verification, allowing you to submit requests with ease on a computer, tablet or smartphone. Enjoy the convenience of this service at no cost. Click here to request your records online and follow the instructions carefully.  This option is particularly helpful for the following individuals to request patient records:

  • Patients requesting specific documents of their own records
  • Parents of minor patients requesting records
  • Caregivers acting on behalf of a patient (i.e. Power of Attorney)

The Online tool verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

Submit Records Requests Through one of The Griffin Patient Portal

To obtain online access to your medical records, you can set up and use the Griffin Faculty Physicians Patient Portal, a private portal that allows you to access your medical records if you have been a patient at Griffin Hospital or Griffin Faculty Physicians.
