Geriatric Medicine
Geriatric consultations are available to patients already established with a Griffin Faculty Physician primary care provider.
Common reasons for a referral from your PCP include:
- Memory loss
- Frequent falls
- Gait and balance disorders
- Behavioral changes
- Episodes of confusion
- Wandering
- Safety issues
- Incontinence
- Multiple medications or medication management
- Decline in self-care abilities
- Increased caregiver dependence, institutional placement being considered
- Frequent doctor or hospital visits

Geriatric Medicine
Ms. Ade is an Advanced Practice Registered Nurse with over 30 years experience in healthcare. She is a graduate of Saint Joseph College where she earned her BSN, and the University of Connecticut, where she earned her MSN, Clinical Nurse Specialist in Gerontological Nursing. In addition to providing geriatric consultation for GFP patients, she also oversees care of hospice patients at Griffin Hospital
